Making the Right Choice for Your Family
When it comes to selecting a skilled nursing facility for yourself or a loved one, we understand that the decision-making process can be daunting. At Tuscany Village, we are committed to making this process as smooth as possible with the help of our professional staff. Our primary goal is to empower families with the knowledge to make the best choice for their loved ones.
Below are the steps taken for a successful admission to a skilled nursing and rehabilitation facility:
Physicians Order
Every potential patient must have a physician’s order for admission, whether they are currently in the hospital or not. Our Medical Director can conduct an evaluation and, if necessary, issue the required order
Patient Information
To better understand individual needs and facilitate the admissions process, we require the following information:
History & Physical (H&P) – provided by your doctor or hospital
Insurance cards (Medicare, Medicaid, BC/BS, AARP, etc.) – please provide copies.
Social Security Card
Copies of Advanced Directives, such as living will, medical power of attorney, DNR (Do Not Resuscitate), etc.
Name, Address, Phone Number of Responsible Party
If your loved one is transitioning from the hospital, most of this information will be supplied by the hospital’s Discharge Planner.
Many of our residents have been referred by hospital Discharge Planners. They are an invaluable resource and we work closely with them, but the ultimate choice in facility lies with the family.
